Purchase Order Standard Clauses
(1) GENERAL
Transcription of certification data is prohibited. Statements on certification documents shall be positive and unqualified. Statements such as “to the best of our knowledge" or “we believe the information contained herein is true" are not acceptable.
Supplier must notify L.M. Gill of nonconforming processes, products, or services and obtain approval for their disposition.
Supplier must ensure that persons are aware of (1) their contribution to product or service conformity and (2) their contribution to product safety.
(2) VENDOR INSPECTION SYSTEM; MIL-I-45208
All materials delivered on this order must be produced under a Quality Management System in accordance with ISO 9002 with the calibration system requirements of ISO 10012 or ANSI Z-540. A quality system meeting MIL-Q-9858 or MIL-I-45208 with the calibration system requirements of MIL-STD-45662 is acceptable as an alternate. Seller's quality control system is subject to review by L. M. Gill Welding & Mfg.
(3) TEST REPORTS OF CHEMICAL AND MECHANICAL PROPERTIES.
One copy required. Original Test Reports showing actual results of all chemical and mechanical properties required by the specification for each heat or lot of material furnished are required. The Test Reports shall be from the original producing mill or when tested by an outside laboratory, on the testing company's letterhead. Test reports are to be identified to our purchase order and contain the material specification/revision including composition, type, class, form, finish grade or condition as applicable to the material supplied.
When two or more heats or lots or analysis are shipped at one time, the packing list and test reports shall indicate the quantities shipped against each heat or lots or analysis.
All documents must bear the typed name, title, and the signature of the person authorized to sign the mill test report. Electronic signatures not permitted unless approved by Electric Boat.
Recertification of mechanical properties is required for each lot/load if a metalworking process and/or heat treatment, forming, etc., is used during fabrication that alters the original condition of the material. The altered material shall be uniquely re-identified for each lot/load.
The Seller shall furnish a certification that the test reports represent the actual attributes of the items furnished on the P. O. and that the results are in a full compliance with the applicable specifications and other order requirements. The certification shall bear the P. O. #, heat/lot# and the item #s and shall be signed and dated by the Seller's authorized representative.
(4) MATERIAL COUNTRY OF ORIGIN © (Must be on Certification)
Only domestic material may be supplied on this purchase order or material may be a NATO country as listed on FAR 225.872-1
(5) CERTIFICATE OF CONFORMANCE
A certificate of conformance is required. The Certificate of Conformance shall be on the vendor's letterhead. The Certificate of Conformance must contain our P. O. #, item, description, heat/lot #s, and material specification as applicable. When material is supplied by L. M. Gill, the certificate shall state that only L. M. Gill supplied material was used to fabricate the customer's product. The Certificate of Conformance must bear the typed name, title and signature of the person authorized to sign.
(6) MERCURY FREE CERTIFICATION
Contamination by mercury in any form is prohibited. All material supplied on this order shall be free from all mercury contamination. Certification is required. The certification shall be on the vendor's letterhead. The certification must bear the typed name, title and signature of the person authorized to sign.
(7) TEST REPORTS FOR PERFORMANCE OF NON DESTRUCTIVE TESTS ©
When nondestructive testing is required (LP, MT, RT, VT or UT) test reports showing the result of all tests performed are required. Test reports shall include: NDT method(s) used. Description or unique identification of item inspected. Procedure Information. Performing Activity. Quantity inspected, accepted, rejected. Acceptance standard used. Date of initial inspection and Inspector name/identification and signature.
As applicable: Penetrant MFR and Type. Instrument MFR and Model/Serial Number. Transducer size & type, search beam angel, and test frequency. Couplant used/Calibration standard number. Technique data must be provided if not included on certification.
(8) MARKING – GENERAL
Complete traceability of the material to the certifications must be maintained. Any material not so marked will be subject to rejection and return. Marking may be labels, permanent markers, etching, stamping or any other method allowed by MIL-STD-792.
MARKING OF CASTINGS
Seller shall ensure the foundry heat number, manufacturer's identification and material specification are marked on each individual casting. Each piece not so marked will be subject to rejection.
MARKING OF FORGINGS
Seller shall ensure the forgings heat/lot number, manufacturer's identification number are marked on each individual forging. Each piece not so marked will be subject to rejection.
NONDESTRUCTIVE TEST MARKINGS
After each casting has satisfactorily met the required standards, it is to be permanently marked as follows:
RT for radiographic test, and the serial number for identification of the castings to the radiographs.
MT for magnetic particle inspection.
PT for liquid penetrant inspection
UT for ultrasonic inspection.
Marking must not reduce the wall thickness of the item below the specified minimum. The depth of the impression must be at least .0003" but not greater than .010"
Marking shall be applied by either of the following methods in accordance with MIL-STD-792: Type I Vibrating tools with rounded (.005min. radius) carbide tip or equivalent. The tool tip shall not be cadmium plated. Type V Stamping with round bottom low stress stamps on a raised pad specifically designed for stamping.
(9) MARKING-SUB-CONTRACTED OPERATIONS
When the marking on a part or piece of material will be removed by any sub-contracted manufacturing process, the marking is to be re-applied in the location and by the method specified in the Purchase Order. Items too small to mark or items that continually have their markings removed by the various manufacturing operations can be controlled by the use of tote boxes, bags, etc., identified with the proper traceability information provided the traceability is maintained at all times. In all cases the accompanying paperwork, (traveler, purchase order, route sheet, etc.) shall provide accountability (qty. cut, rejected, scrapped, etc.) and the traceability throughout the manufacturing process.
(10) INCOMING INSPECTION & PARTIAL SHIPMENTS
All materials and supplies shall be subject to L. M. Gill incoming inspection to ensure that quality assurance provisions are met.
Partial shipments without prior approval by L. M. Gill and shipments without required certification shall be subject to rejection.
(11) SPECIAL REQUIREMENTS FOR NICU, NICUAL AND CUNI ALLOY PARTS
The sulfur content of the gas and oil fuels used in firing furnaces for forging, extruding, or other hot working process and for heat treatment of NICU (N004400), NICUL (N00500), CUNI (C71500), and other nickel base alloys shall be limited as follows:
Gas: 30 grains per 100cu. Ft. maximum
Oil: 0.5% by weight maximum
(12) GOVERNMENT SOURCE INSPECTION (GSI)
Government inspection is required prior to shipment from your plant. Upon receipt of this order, promptly notify that Government representative, or if none, the nearest DCASMA officer, who normally services your plant so that appropriate planning for government inspection can be accomplished. In the event the representative or officer cannot be located, our purchasing agent should be notified immediately. FAR 52.246-2 applies.
(13) DPAS RATING SYSTEM
Certified National Defense use under DPAS. The priority rating is:___________
(14) GOVERNMENT/CUSTOMER RIGHTS FOR REVIEW
During performance of this order, your quality control and/or inspection system and manufacturing processes are subject to review, verification and analysis by authorized Government and/or End Use Customer at any level of the supply chain, L.M. Gill representative upon his request.
(15) EB 2678N
All materials delivered under this Purchase Order must be produced I/A/W and meet the requirements of General Dynamics Electric Boat Division Specification 2678N.
(16) SHELF-LIFE REQUIREMENTS APPLY Material supplied on this order requires a Cure Date (rubber products) and Expiration Date per the material specification being supplied. Material shall not have more than 1/3 of it's expired shelf-life at time of shipment. Material received not complying with the following are subject to rejection. Ref; MIL-HDBK-695.
(17) MALPRACTICE AWARENESS
Contract Compliance and Awareness of Malpractice Prevention per EB Specification 2678N, Appendix B is a part of this order. Copies of the appendix are available upon request for your file.
(18) NOFORN
No foreign Nationals are allowed to review the technical information or data, process or work on this order. Foreign National means any person, not a U.S. citizen or national of the United States. Typically, green card holders are Foreign Nationals. Contact L.M. Gill's Security Officer immediately if you have any questions on this NOFORN requirement.
(19) No Weld Repair Allowed. (Must be stated that "No weld repair was performed" on Cert)
(20) Requirements invoked by the Purchase Order shall be passed on to all sub-tier suppliers including key characteristics where required, customer requirements, and customer designated sources.
(21) L.M. Gill shall be notified of any changes in product and/or process, suppliers and manufacturing facility location and shall obtain organization approval (when required).
(22) CALIBRATION REQUIREMENTS
The Following requirements are required as shown.
Qualified personnel perform calibration activities under controlled conditions.
Utilized instruments/masters are certified and traceable to the National Institute of Standards and Technology (NIST) or other national recognized standards.
Must affix a sticker indicating the person performing the calibration, the date of calibration, the re-calibration due date, and the IM&TE identification.
Notify and report to L.M. Gill all "as-received" values if out of tolerance.
Affix tamper-proof seals as appropriate.
Comments, if any.
(23) Record retention of objective quality evidence shall be seven years. L.M. Gill approval is required prior to destroying records after the seven (7) year retention period. ©
(24) All materials provided under this purchase order must be free of conflict minerals in accordance with the Dodd Frank Act Section 1502. ©
(25) COUNTERFEIT PARTS PREVENTION ©
Supplier shall guard against the use and delivery of counterfeit parts or components to L.M. Gill
(26) FOD (FOREIGN OBJECT DEBRIS/DAMAGE) CONTROL ©
Any item that could/would be classified as FOD (Foreign Object Debris/Damage) shall not be used in the preparation of packaging of product, or parts delivered to L.M. Gill
